Build or edit an intake form
Updated 25 September 2026
Intake forms collect information from a client before you start working together.
- Go to Intake Forms and click + New intake form.
- Enter a Form name and click Create form.
- Open the form and click Edit questions to add to it.
For each question, enter a Question label, choose a Type β Mapped (writes to a profile field), Freeform (an intake-only answer), or π― Goals β and tick Required if the client must answer it.
A Mapped question can write to a built-in field or one of your custom fields marked Fillable via intake form. A Freeform question can be short text, paragraph, date, single select, multi-select or a 1β10 scale.
- Click Add question.
Opening a form shows the client's-eye preview by default; click Edit questions to see and reorder the question list, and Done editing to return to the preview.
From the form's header you can Rename it, Duplicate it, or (for forms you created) Delete form β deleting also un-attaches it from any Session Type using it. Coachstra's own starter template is marked Coachstra template and can't be deleted, only duplicated into a version you can change freely.
